Referral Process: 

1. Find the qualifying patient

2. Need doctors order for inpatient evaluation

3. Types of documentation needed:

   1. Face sheet

   2. Health & physical

   3. All progress notes

   4. All consults

   5. Operative reports

   6. Most recent vital signs

   7. Most recent labs

   8. Any culture reports

   9. An active MAR

   10. Imaging

   11. PT, OT, & ST notes

4. Fax over the referral (225)450-2813

5. A clinical liaison will contact you within two hours.

 

**A doctor will call for a home patient or a social worker will call for a hospital inpatient.
